Care Management

Our professional Care Managers are credentialed experts in geriatric care. Our Care Managers work closely with our clients and their families to assess their specific situation and offer guidance through a custom plan of care. Every family situation and client is unique and that is where the plan of care helps to communicate this information.

The plan of care provides day-to-day and long term care goals for the client and provides a convenient document for sharing this information with the client and family. The plan of care is also reviewed with your family member’s Caregiver and our Care Manager on a regular basis to ensure that goals are being met. The plan of care is integral to communication between all parties involved in caregiving for that client.
